Distinguished Alumni Award Nomination Form

To be eligible for consideration, the nominee must:

• Be a graduate of Hudson Valley Community College.
• Have demonstrated outstanding professional achievement and distinguished success in their chosen career or field.
• Exhibit exceptional integrity, leadership, and character that reflect positively on Hudson Valley Community College.
• Have made meaningful contributions through civic, charitable, educational, cultural, military, or community service.
• Serve as an inspiration to current and future Hudson Valley Community College students through their accomplishments and leadership.
• Be willing to participate in recognition activities if selected.

Preference may be given to nominees who have demonstrated continued engagement with or support of Hudson Valley Community College through volunteerism, mentoring, advocacy, philanthropy, or participation in alumni activities.

Anyone may nominate a Hudson Valley Community College graduate for the Distinguished Alumni Award.

To submit a nomination:

• Complete this nomination form in its entirety.
• Include a detailed statement describing why the nominee deserves this recognition.
• Attach supporting documentation, such as:
  • Résumé or biography
  • Professional accomplishments and awards
  • News articles or publications
  • Letters of support (optional but encouraged)
  • Examples of the nominee's service to Hudson Valley Community College and/or their community

Please provide a detailed narrative describing why the nominee should be considered for the Hudson Valley Community College Foundation - Distinguished Alumni Award.

Nominees will be evaluated based on the following criteria:

• Professional Success – Outstanding achievements, innovation, and leadership in their profession or career.
• Civic Affairs – Meaningful contributions to public service, civic organizations, or initiatives that strengthen communities.
• Cultural Affairs – Contributions that promote the arts, education, diversity, heritage, or cultural enrichment.
• Charitable Affairs – Demonstrated commitment to philanthropy, volunteerism, nonprofit organizations, or service to others.
• Leadership and Integrity – Exemplary ethical leadership, character, and conduct that reflect positively on Hudson Valley Community College.
• Contributions to Hudson Valley Community College – Meaningful engagement with the college through volunteer service, mentoring, advocacy, alumni involvement, philanthropy, partnership, or other activities that have strengthened the institution and benefited its students.
