Contact Tracing Log Information

Per Rensselaer County Department of Health’s request, Hudson Valley Community College is instituting a contact log to assist the county in contact tracing in the event of a COVID-19 exposure.

Employees in offices or service areas that serve students, faculty, staff or customers must fill out the form each time a new individual who does not work in your immediate area visits your office or service area. Please note that this process does not apply to classrooms.

The process should take only a brief moment to complete. Please see the instructions below.

The employee will log into the Contact Tracing Check In page to fill out the form on behalf of the person entering the office or area. No physical contact is required. The form will be filled out differently depending on whether the individual has a Hudson Valley ID badge.

  • If the person entering the office has a Hudson Valley ID badge: Swipe the badge (if your office has access to a swiper) or manually type in the person’s college-issued H00#. This will populate the First Name, Last Name and Date of Birth fields automatically. Then, fill out the building and the room number the individual is visiting and select Submit.
  • If the person entering the offices does NOT have a Hudson Valley ID badge or H00#, manually fill out the form by asking the person for their First Name, Last Name, and Date of Birth. Then, fill out the building and the room number the individual is visiting and select Submit.

After selecting Submit, a message will appear that the form has been submitted and the webpage will automatically redirect back to the original form – ready to be filled out again – after a few seconds.

The form will automatically log the username of the employee who is submitting the form and the date/time the form was submitted.
